New Patients

If you live within our catchment area – you will be registered as a regular patient

If you live outside our catchment area – you may be able to register here as an Out-of-Area patient, which means there will be some restrictions to certain services we can provide to you.

How to Register

Joining the practice is easy. All eligible people living in the practice’s catchment area can register with Chelsfield Surgery as a regular patient.

If you do not have an address of your own, please state upon the new patient form that you do not have a fixed address.

You do not need to provide proof of your address, photographic ID or immigration status to register with us.

**Please note: If you have provided a mobile number, we will text you a welcome message once we have accepted and added you to our system. We aim to get new patients registered within 4 working days after your submission. If you do not receive a text after 4 working days, please contact us to find out if you are registered. If there is a problem with your registration, we will contact you as soon as possible to ensure there are no delays in registering you**
