New Patient Registration
Our doctors welcome new patients who live within our practice catchment area. You can register by completing a registration form which can be collected from the surgery or downloaded below for completion at home before coming into the surgery.
If you are HOMELESS, or of NO FIXED ABODE, you can register at your GP surgery with no proof of identity or proof of address papers, however if you have these documents it is always helpful for our records.
Please visit the surgery after 2pm with your completed forms or proof of ID, when the surgery is less busy, OR registering ONLINE via this website and then presenting ID proof in person or via email.
When registering, please bring with you the following for yourself and any other persons registering with you:
- Your NHS number (your previous GP practice will be able to give this to you if you have lost your NHS card).
- Photo idenfication (eg passport, driving licence, student card)
- Proof of address (eg utility bill, bank statement dated within the last 3 months)
- If you are registering a child, please ensure you bring their red Health Book (please note that in order to register a child with the practice at least one parent/guardian must also be registered with us).
You will be asked to complete a registration form for each person registering. In addition, all patients will be asked to complete a New Patient Confidential Health Questionnaire, allowing us to provide medical care in the interim period while your medical records are transferred from your old practice.
Once registration forms are complete and signed and relevant proof of identification/address has been seen, new patients will be registered and medical treatment/appointments will be available.
Once you have been accepted as a patient your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving.
You may print off a Registration Form and New Patient Confidential Health Questionnaire, fill it out and bring it in with you to the practice.
