Some services provided are not covered under our contract with the NHS and therefore attract charges, for example:
- Medicals for pre-employment, sports and driving requirements (HGV, PSV etc)
- Insurance claim forms
- Prescriptions for private medications (e.g Malaria tablets)
- Private sickness certificates
- Travel Cancellation Forms
Our secretaries cannot produce 'to whom it may concern' letters for leisure park entry, or dispensation letters for relatives to visit from abroad or vocational trips in respect of patient's current health or medication. We can only supply if the person requesting the letter has permission to discuss medical record, a copy of the patient record and nothing more, to which no charge will be made.
The fees charged are based on the British Medical Association (BMA) suggested scales:
Private Insurance Claim Form
Medicals (dependent on form)
All Private Prescriptions
Private Administration Letters
More detailed reports/letters will be charged at a higher rate
Unfortunately our GPs are no longer able to sign passport forms for patients, or do 'FIT TO PARTICIPATE' letters.
Payment can be made either by cash, cheque or debit card (we do not accept credit cards) and will be required on commencement of the vaccine course. Please make cheques payable to Chelsfield Surgery.
We now have a card machine in the surgery for payment of these chargeable services by your debit card. Please ask reception if you wish to use this facility.
